Palliative Plan of Care

Palliative care helps people with a life-limiting illness to have the best possible quality of life. It is available for everyone – of any age, race, culture, background or religion. With our Palliative plan of care, our medical staff is devoted to your care and comfort, and we provide you with a tailored plan of care and keep you informed every step of the way. Bereavement support and Counseling

We understand that Hospice can be an overwhelming experience for everyone involved. We have Chaplains available to provide spiritual support if needed to the patient, the patient's families and caregivers. We offer Bereavement support for your family and loved ones throughout your illness and for up to 12 months following the end of life.

Pain Management

We strive to keep the patient comfortable and manage symptoms to ensure they have the highest quality of life for as long as they live. ... Our hospice care team will evaluate a patient's pain on every visit using pain rating scales and their expertise.

Medications and Medical Equipment

We cover most Medications and Medical equipment as needed. Medications are delivered when needed as well.

24 Hour On Call Skilled Nursing

When you or a loved one has to make a trip to the hospital, every minute can feel like an hour. Our team is available 24/7 because we know a crisis can happen at any time. We are here for any concern or questions at any time.

Volunteer Services

Volunteers play a vital role. They enhance the quality of life of the patient in many ways. They provide compassionate support for patients and their families in home, and assisted living facilities, and skilled nursing facilities. They're able to keep patients company when family and friends are not able to be there, and much more. They help provide a high level of patient care.

Licensed Social Workers

Social Workers are available for psycho social, emotional support, community services and resources.
